Assumption of Risk and Waiver of Liability

I, {name}, hereby acknowledge and agree that participation in martial arts, grappling, and fitness training activities at Rolling Oak Jiu-Jitsu Academy LLC involves inherent risks, including but not limited to:

• Physical injury, sprains, strains, or fractures.
• Emotional stress or mental fatigue.
• Accidents during training, practice, or live sparring sessions.

I voluntarily assume all risks associated with these activities and agree to release Rolling Oak Jiu-Jitsu Academy LLC, its owners, instructors, staff, and affiliates from any and all liability, claims, or demands resulting from participation in academy programs, whether caused by negligence or otherwise.

Medical Disclosure

I certify that:

• I am physically and medically fit to participate in martial arts activities.
• I have disclosed all relevant medical conditions that may affect my ability to safely participate in training.
• In the event of a medical emergency, I authorize Rolling Oak Jiu-Jitsu Academy LLC to secure appropriate medical treatment for me or my child if the need arises.

Code of Conduct

I agree to adhere to the academy’s rules and policies, respecting instructors, staff, and fellow students. Disruptive or inappropriate behavior may result in termination of membership without a refund.

4. Assumption of Risk and Liability Waiver
